Hellos and paperwork
- Please come in 15 minutes before your appointment so you have time to complete your paperwork and discuss and necessary insurance questions.
- Save time: Fill out you paperwork at home. Scroll to the bottom of this page and download the “New Patient Docs”. By downloading and filling out your forms now you can save time in the office later.
Initial comprehensive exam & X-rays
You will meet with your dentist or dental hygienist to talk about your medical history. If you have any oral health worries or fears, you can feel comfortable sharing them with her—your comfort is most important, so we encourage you to ask questions.
Next, we will likely examine your teeth, gums, and mouth, and take x-rays and/or panoramic radiographs. Panoramic radiographs help your dentist view unerupted or impacted teeth, as well as sinus conditions, and can detect lesions or fractures in the jaw. Please notify our staff if you have had x-rays taken within the last 3 years so we can let you know if they are needed at your scheduled appointment. WE WILL NEED CURRENT X-RAYS TO COMPLETE A COMPREHENSIVE EXAM AND DETERMINE WHAT HYGIENE TREATMENT IS SUITABLE FOR YOU.
- This initial comprehensive exam is a no-risk way to learn the status of your oral health, and to learn how our dental team can help you.
- There’s a good reason why you may not have your teeth cleaned at your first appointment: Each new patient relationship often starts with an initial exam so your dental team can understand your needs and concerns first, then tailor your hygiene treatment to you. This way, you’ll get the most out of your cleaning appointments and the time and attention you deserve.

Payment
We accept payment by credit card (Visa, MasterCard, Discover, American Express) as well as by check or cash.
We offer a financing program with Care Credit. More information can be found at our office.
Forms
Save time by completing your forms at home
We value your time, and hope to make your first visit convenient and easy. You may optionally complete the “Financial Policy” form and the “Patient Information” form by downloading them below. Please print the forms, complete them at your convenience, and bring them with you when you visit the office. Please let our receptionist know so that we can ask you to come at the optimal time.
